    Location: Catholic Community Services of Utah, 745 E 300 S SLC UT 84102 Status: Non-Exempt, Full-time, with benefits Salary: $14.50-15.50/hr DOE Catholic Community Services of Utah provides help and creates hope for thousands of people of all beliefs who are most in need in our community through Migration and Refugee Services, Homeless Services and CCS of Northern Utah programs. Job Description: Provide case management services as outlined in the Preferred Communities Grant, state and federal regulations, and in compliance with policies and procedures of Catholic Community Services Refugee Resettlement Program. Responsibilities include: 1. Manage medically vulnerable cases through Intensive Case Management. 2. Coordinate extensive medical appointments and interpretation for clients. 3. Help eligible clients apply for Supplemental Security Income (SSI) and appeals collaborating with community resources for attainment of clients SSI benefits. 4. Collaborate with community resources for ADA housing, medical equipment procurement, and ADA transportation. 5. Provide referrals, when needed, for medically vulnerable individuals in need of social, interpersonal, emotional, or vocational resources. 6. Conduct needs assessments for newly arriving medically vulnerable refugees, as well as determining when clients will be assisted internally versus being connected to outside sources of treatment. 7. Other duties as may be assigned. Work Environment – inside/outside, office, warehouse, noise, temperature, etc. 1. This position is in a professional office environment using standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. 2. Some local travel is required. Physical Demands including bending, sitting, lifting and driving 1. This is largely a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets, and bend or stand as necessary. 2. This role includes driving locally to apartment complexes and meetings. 3. Must be able to lift at least 50 lbs. Skills, knowledge and abilities: 1. Works independently in safe, appropriate manner. Demonstrates both problem solving and problem prevention. 2. Displays sensitivity to the needs of the youth, families, CCS staff, volunteers, outside agencies and other persons with whom the employee may interact. 3. Adheres to deadlines. 4. Proficiency in English and Swahili languages-oral and written communication. 5. Proficiency in computer skills, including Microsoft Office, that are required for the position. Education, training and or experience: 1. Bachelors of Social Work, Public Health, or Health Sciences from an accredited institution or equivalent experience. 2. Experience and/or training required in working with medically vulnerable individuals. 3. Experience of working with people from diverse cultures and different ethnic background is necessary. Background Screen/BCI requirement 1. Must pass BCI/FBI background check. Licenses, certifications and credentials 1. Current Utah driver’s license, proof of auto insurance with minimum policy liability limits of $50,000 per person and $100,000 per occurrence, 21 years of age or older, a clean driving record, and ability to meet CCS vehicle safety requirements. Position status and expected schedule 1. This is a full-time, 40 hours per week position. 2. Monday through Friday 8:30 am to 5:00 pm, sometimes past 5:00 pm if a night arrival is scheduled.
